Signs a Student May Have Underlying Visual Skill Deficits Affecting Math

There is a pattern I see repeatedly in schools that often gets misunderstood.

A student can be:

  • intelligent

  • verbal

  • and conceptually strong

And still struggle significantly in school.

Not because they don’t understand.

But because access is breaking down at the visual level.


And this is what it looks like with Math.


What this may look like

A student may:

  • understand math verbally but struggle on paper

  • reverse numbers or misalign columns

  • lose their place during multi-step problems

  • struggle copying from the board

  • avoid math despite strong intelligence

  • fatigue quickly during math tasks

  • struggle with mental math

  • find geometry or spatial reasoning overwhelming

  • skip steps in problem solving

  • show inconsistent output

  • struggle with organization on the page

  • need excessive time to complete work

  • become overwhelmed by word problems


Individually, these may not raise concern.

Together, they often point to something deeper.


Common Misinterpretations

These patterns are often labeled as:

  • lack of focus

  • poor effort

  • carelessness

  • motivation issues

  • “not a math person”


But from an OT and visual processing perspective, these can reflect breakdowns in:

  • visual perception

  • visuospatial processing

  • visual efficiency


Why tutoring alone sometimes isn’t enough.

Tutoring absolutely matters!


Practice matters.


Instruction matters.


But if the access system is inefficient, students may still be:

  • working harder than necessary

  • using more energy to organize the page than to think

  • losing cognitive bandwidth to visual demands

  • burning out before they can show what they know


Which can lead to:

  • inconsistent performance

  • fatigue

  • frustration

  • avoidance

  • low confidence


This isn’t a motivation issue.


And it isn’t always an instruction issue.


Sometimes it’s an ACCESS issue.


The Role of Visual Systems in Learning


Math requires continuous coordination of:

  • visual tracking

  • spatial awareness

  • visual memory

  • sequencing

  • attention control

  • working memory


When these systems are strained, math becomes significantly more effortful than it needs to be.


What this means for school-based OTs and educators...


If you’re a school-based OT, this probably feels familiar.

Because this is where the question gets harder:

Is this OT? instruction? tutoring? attention?

And often, we’re asked to make sense of that fast.


What I find helpful is shifting the lens:

Instead of only asking “What is the student doing?”

We can also ask “What is the student having to manage just to access the task?”


That’s often where OT sits:

  • visual organization on the page

  • tracking across space

  • visual-motor load under cognitive demand

  • breakdowns that only appear when tasks get complex


Not as the “math fix.”


But as the lens that helps separate ability from access demand.


The reframe

I keep coming back to this question:


What is it costing the student just to access the task?


Because when access is expensive, performance becomes inconsistent—even when ability is not in question.
